Who is eligible to participate?
This incentive program is open to all State employees, pensioners, and their dependents covered under a State medical plan.All you need is a current State healthcare plan.

How much does it cost?
Not only is there zero cost to participate in the NGH Incentive Program, but you’ll also enjoy $0 copay for healthcare at Nashville General Hospital (NGH) and Nashville Healthcare
Center (NHC).2
How do I sign up?
You don’t! There’s no sign up needed. If you currently have a State healthcare plan, you’re eligible NOW! New patients call 615.341.4YOU (4968) to book your appointment.

You will continue to recieve an Explanation of Benefits (EOBs). The EOBs will state “amount owed provider” — this is not a bill. This is the amount we do not collect unless it is referencing a third-party vendor as previously noted.

Group Plan Specifics
- 1 Represents out-of-pocket maximum of $2,000 single and $4,000 family for BlueCross BlueShield (Network S & P), or Cigna (LocalPlus or OpenAccess) in-network providers. You will NOT incur any copays for healthcare at NGH, NHC, and NHDI.
- 2 To qualify for no-cost care under the Nashville Healthcare Incentive Program, all third-party labs and outside imaging/rehab services must be performed at Nashville General Hospital, Nashville Healthcare Center, or Nashville Healthcare Diagnostic Imaging, even if employees are seeing a physician at Nashville Healthcare Center.
- 3 Copays may be charged by a third-party lab.
